I hereby grant TDSoccerPerformanceAcademy LLC, the right to use photographs, Facebook live, Instagram live, YouTube live, video images and/or other media of my child for publicity, advertising and/or other commercial purposes.

By signing this release and by being enrolled in this program you assent to the enforcement of this policy.

Dates, times and prices are subject to change. I understand that this document is intended to be as broad and inclusive as permitted by the laws of the county, state, and country in which the Event and/or instruction is taking place and agree that if any portion of this agreement is invalid, the remainder shall continue in full legal force and effect.

CANCELLATION POLICY: Cancellations for any TDSoccerPerformanceAcademy LLC will result in no refunds. TDSoccerPerformanceAcademy LLC, reserves the right to cancel or change program dates, times and/or locations.

NOTE: TDSoccerPerformanceAcademy LLC cannot guarantee results. All players progress at different rates and with different intensities.
